miDASH provides a platform for the secure storage of, and access to, your personal health information. In the course of our activities in providing this service, miDASH may collect, use and disclose Personal Information. To demonstrate our commitment to your privacy, miDASH has prepared this policy statement to inform you how we endeavour to ensure the privacy and confidentiality of your Personal Information.

This privacy policy governs your use of the software application miDASH (“Application”).


When you use miDASH's services and you access physician-validated personal health information, you are the custodian of that personal health information. miDASH has possession of that information, in your personal health record on our servers, but has no independent interest in such information. miDASH is not a custodian of this information and is not a "health information custodian" within the meaning of health privacy legislation. miDASH does have custody of other personal information. ("Personal Information"). This Personal Information consists of identifying and billing-related information as well as any information about our interactions with you while you are a miDASH client or service provider. Examples of personal information stored in miDASH for validation include first name, last name, address, email address, phone number, and HIN.


miDASH generally will not disclose your Personal Information unless required to do so by law or to provide you with our services. miDASH may disclose, with your express written consent, your Personal Information to individuals or organizations who have or may need to:

  • Establish a relationship with you;
  • Provide you with our services and ongoing support;
  • Provide you with information about new or existing services;
  • Process or collect fees for services;
  • Support core business functions within miDASH;
  • Meet legal and regulatory requirements; and
  • Such other reasons consistent with these purposes.

We endeavour to collect information directly from you as much as possible but may also receive Personal Information about you from other sources (e.g. from your physician; from references for individual service providers). We receive and store certain types of information when you interact with our website. Our purpose in doing so is to allow our website to work correctly, to evaluate its use, and to support website analytics and marketing campaigns. Such information may include technical information such as your internet protocol address, your computer's operating system and browser type, the address of a referring website, if any, and the path you take through our web pages. Our use of Personal Information is limited to the purposes described in this Policy, or consistent purposes. miDASH does not otherwise sell, trade, barter, exchange or disclose for consideration any Personal Information.


miDASH generally will not disclose your Personal Information unless required to do so by law or to provide you with our services. miDASH may disclose your Personal Information to individuals or organizations who are or may be:

  • Our advisers or service providers;
  • Involved in a transfer of all or part of the assets or business of miDASH;
  • Maintaining, reviewing and developing our business systems, procedures and infrastructure.

Where miDASH discloses Personal Information to individuals or organizations that perform services on its behalf, we will require those service providers to use such information solely for the purposes of providing services to miDASH or you and to have appropriate safeguards for the protection of that Personal Information. Where Personal Information may be subject to transfer to another organization in contemplation of a merger, sale of assets or business units or change of ownership, we will do this only if the parties have entered into an agreement under which the collection, use and disclosure of the information (including any Personal Information) is restricted to those purposes that relate to the business transaction. Please note that there are circumstances where the use and/or disclosure of Personal Information may be justified or permitted or where miDASH is obliged to disclose information without consent. An example of disclosure without consent is where it may be required by law or by order of a court or governmental tribunal. Where obliged or permitted to disclose information without consent, miDASH will not disclose more information than is required. By providing personal information and personal health information to miDASH you agree and consent that we may collect, use and disclose your information in accordance with this policy.


miDASH uses service providers or suppliers that help with our business operations who may access or store your Personal Information. Examples are billing and refund vendors, credit card processors, and companies that help us improve our product and service offerings and our web sites. We require these service providers and suppliers to keep the information secure. We also prohibit them from using your information for any purposes other than those requested by us. Please note that your Personal Health Information can only be accessed by you, your physician and anyone else specifically authorized by you. Your information is protected using strong encryption. A third-party service provider cannot access your personal health information unless done illegally.


While miDASH is the custodian of the Personal Information provided to us, some associated services are provided by third party service providers. While your personal information is stored in Canada, some of our service providers and the databases where they store personal information (other than your personal health information) may be located in the United States of America. Please note that Personal Information located in any jurisdiction is subject to the laws of that jurisdiction.


Unless permitted by law, miDASH will collect no Personal Information without obtaining the consent of the individual concerned. However, we may seek consent to use and disclose Personal Information after it has been collected in those cases where we wish to use the information for a new or different purpose. By providing personal information and personal health information, you agree and consent that we may collect, use and disclose your information in accordance with this policy. In addition, where appropriate, specific authorizations or consents may be obtained from time to time. If you choose not to provide us with any required Personal Information, we may not be able to offer you our services. If you choose to withdraw your consent, which you are free to do, we will inform you of any consequences in doing so.


miDASH endeavours to ensure that any Personal Information in its custody is as accurate, current and complete as necessary for the purposes for which we use that information. Information in your personal health record is verified so as to ensure its accuracy and completeness at the time of entry. We keep Personal Information in our custody only as long as it is required for the reasons it was collected. The length of time we retain information varies, depending on the purpose for which it was collected and the nature of the information. This period may extend beyond the end of your relationship with us but it will be only for so long as it is necessary for us to have sufficient information to respond to any issues that may arise at a later date. Personal Information in our possession (i.e. the personal health information in your miDASH record) is kept for 20 years from the date of the creation of your record. When Personal Information in our possession or custody is no longer required for miDASH's purposes, we have procedures to destroy, delete, erase or convert it into an anonymous form. All information in miDASH's custody or possession is held in Canada.


miDASH endeavours to maintain appropriate physical, procedural and technical security with respect to its offices and information storage facilities so as to prevent any loss, misuse, unauthorized access, disclosure, or modification of Personal Information in our custody or possession. This also applies to our disposal or destruction of such information. miDASH protects Personal Information by restricting access to it to those employees or service providers that miDASH has determined need to know that information to allow miDASH to provide our services. Any employee misuse of Personal Information will be considered a serious offence for which disciplinary action will be taken, up to and including termination of employment. Where an individual or organization is provided Personal Information in order to provide services to or for miDASH, any misuse of such information will be considered a serious issue for which action will be taken, up to and including termination of any agreement between miDASH and that individual or organization.


With respect to personal health information in your record, you may access it at any time. Where Personal Information is in the custody of miDASH, we permit a reasonable right of access and review and will endeavour to provide the information in question within a reasonable time, generally no later than 30 days following the request.


miDASH allows access to family members accounts with an email consent to accept or reject from the family members. In agreement with the policy, you may not use or disclose identifiable or anonymized personal information to any third party without the express written consent of the individual to whom it refers.


miDASH will make this policy statement available on its web site as well as other places that miDASH’s management determines appropriate. miDASH may, from time to time, conduct privacy impact assessments on its service offerings, and will make summaries of such assessments available upon request.


Use of Synapse may be available in your area. If enabled, you acknowledge that You will use Synapse for the purpose of communicating with your health provider regarding management of your health. (“Purpose”). If You breach any provision of these Terms of Use or You engage in any fraudulent, abusive or otherwise illegal activity, we may, in our sole discretion, restrict, suspend or terminate Your access to the Services without notice to You and without liability to You.

You also acknowledge that you are aware that Synapse is not to be used for providing urgent or emergent medical care by your healthcare provider or health care team. Your healthcare provider will make all efforts to respond to your communication within a timely fashion. Should you have a medical concern that requires urgent or emergent attention you are requested to call 9-1-1 or attend your nearest Emergency Department and not use miDASH Synapse to seek medical attention. You are also aware and agree that responses to messages via the miDASH system are at the sole discretion of the health care provider and You may be asked to come in for an in-person appointment with your healthcare provider. You also are aware that your name and any other details that you provide in your profile may be visible to any other member in the network.


The Pricing and Payment Terms were last modified on October 31,2018. By using the Service, you signify that you have read, understood, and agree to be bound by these Pricing and Payment Terms. 

Capitalized terms that are not defined in the Pricing and Payment Terms have the meaning given to them in miDASH’s Terms of Service. The miDASH Terms of Service are hereby incorporated by reference. 

miDASH accepts certain credit cards and will automatically charge your selected credit card for all applicable fees on a monthly or yearly basis, depending on which Plan you select, until you or we cancel your Plan. The Plan fee will be billed when you first choose to open a Plan and each month or year (depending on which Plan you select) thereafter unless and until you cancel your membership. Plan charges are fully earned upon payment and there are no refunds or credits for partially used months or years, as applicable. If any fee is not paid in a timely manner, or miDASH is unable to process your transaction using the credit card information provided, miDASH reserves the right to revoke access to your miDASH Plan. Provincial taxation will be applied appropriately.

All information that you provide in connection with a monetary transaction through the Service must be accurate, complete, and current. You agree to pay all charges incurred by users of your credit card, debit card, or other payment method used in connection with a purchase or transaction or other monetary transaction through the Service at the prices in effect when such charges are incurred. You will pay any applicable taxes, if any, relating to any monetary transactions.

If you want to use a different credit card or if there is a change in your credit card validity or expiration date, you may edit your information by accessing your Plan setting page. If your credit card reaches its expiration date, your continued use of the miDASH constitutes your authorization for us to continue billing that credit card and you remain responsible for any uncollected amounts. It is your responsibility to keep your contact information and payment information current and updated.

1. NO Refunds

You may cancel your miDASH Plan at any time. However, there are no refunds for cancellation; if you cancel before the end of your billing cycle you have access to your Plan for the remainder of the billing cycle. In the event that miDASH suspends or terminates your Plan or this Agreement for your breach of this Agreement, you understand and agree that you shall receive no refund or exchange for any unused time on a subscription, any license or subscription fees for any portion of the Service, any content or data associated with your Plan, or for anything else. 

2. Cancelling Your Plan

Your miDASH Plan will continue in effect until you cancel your Plan, or we terminate it. You must cancel your Plan before it renews each month or year (depending on which Plan you select) in order to avoid billing of the next month’s/year’s fees associated with your credit card. miDASH will bill the monthly/yearly fees associated with your Plan plus any applicable tax to the credit card you provide to us during registration (or to a different credit card if you change your account information).

3. Changes to the Pricing and Payment Terms

miDASH may, at its sole discretion, modify or update these Pricing and Payment Terms from time to time to reflect the changes in our business and practices, and so you should review this page periodically. When we change the policy in a material manner we will let you know and update the ‘last modified’ date in this text.

4. Special Offers.

miDASH may enable special offers for certain eligible groups. If you qualify and subscribe through a special offer, you will be billed the advertised special rate for the appropriate time period and will be automatically renewed at the regular rate after that.
