1. Can my friends/family register for miDASH too?
    Yes, If your friends/family belong to one of our registered clinics then they can register for miDASH easily. Contact Us if you need any assistance with registration.
  2. I am unable to create an account on miDASH but belong to a clinic that offers miDASH, what do I do?
    You will be able to create an account if all necessary information is in your clinical chart. Often there is missing information such as your cell phone number or email address in your Electronic Medical Record. Please contact your clinic and provide these details and indicate that you are wishing to create an account on miDASH. They will help guide you through the necessary information to create an account. 24 hours after your clinical chart has been updated you should be able to register.
  3. I have children who do not have email addresses, can I add them to my account?
    • If your children are 14 years or younger, please advise your clinic to add them as your family member.
    • If your children are 15 years or older, you will require consent from them to add them to your account.
  4. Can I add my husband, parents or in-laws to my account?
    • Please ask your clinic to add them as a family member in your electronic medical record. You will still require consent from them to view their health record.
    • If you have the Power of Attorney for Personal Care (Ontario) or have been designated as the substitute decision maker for any of them, please advise the clinic to add you to their account in that capacity.
  5. How do I delete my account?
    If you would like to delete your account please send us a message through Contact Us . We will need your instructions along with your full name and username.
  6. How do I change my password?
    Login to your miDASH account. Go to the pull down arrow on the top right of the miDASH profile page. Hover over it and choose "Change Password" option. Enter your current password, new password and confirm new password into the "Change Password" popup window.
  7. Who should I contact if the information in my account is incorrect?
    If you find incorrect information in your miDASH account, please send us a message through Contact Us . We will need your instructions along with your full name,username and the information that you found incorrect.
  8. Can we use the same email to register two separate accounts?
    No, you cannot use one email to register two separate accounts.
